Default settings for APN proxy on Sprint USA?

I thought this app would configure automatically but when I start it up it just says "Waiting... No Valid Connection..."

When I go to the proxies tab and select APN it asks for some settings such as MCC+MNC, Operator, and Proxy Type. Could someone please tell me the default values for use on Sprint USA? I have no clue.

Also for future reference it would be nice to know the default values for WIFI SSID as well.
